- Proof of address
Students must be 5 years of age by October 1 of the year that they are entering kindergarten.

Any disabilities or health problems should be indicated on the registration forms.
No student is permitted to attend or continue to attend any school in the District without meeting the legal requirements of immunization against disease unless the student has a valid exemption for health, religious, personal, or other reasons as provided by law.
Students who do not submit an up to date certificate of immunization or a written authorization signed by one parent or guardian requesting local health officials administer the immunizations or a valid exemption will be suspended and/or expelled from school according to regulation JLCB-R.
When withdrawing your child from school, please give the school office a week’s notice, if possible, so that all necessary records may be prepared for transfer. If your family moves during the school year, or if home or work telephone numbers change, please contact the school office at 328-8930.
Class Placement
At the end of each school year, classroom teachers at each grade level provide valuable information about individual student’s learning needs. The principal uses this information, along with input from parents, to make final student placement decisions. Parents are encouraged to share insights about their child; however, requests for a specific teacher are not taken.
